2023 REGISTRATION FORM

We are excited to open Registration for the 2023 Season.

Currently registration is open for the following divisions;

U19+ Girls (born 2002-2005)
U17 Girls (born 2006-2007)
U15 Girls (born 2008-2009)
U13 Girls (born 2010-2011)

MDSA will play as part of the York North Loop for the 2023 season.

U15, U17 and U19+ are expecting to play Home games on Wednesday and Thursday evenings at Berczy Park South diamond in Markham.

The U13 division are expecting to play Home games on Monday evenings, however a diamond has not yet been confirmed.

Payment is required in order to confirm your registration. After submitting your contact information and player details, please submit payment via EFT or directly from the Online Payment page.
PLEASE NOTE: This is a two-step process and we require both the Registration info from this form in addition to payment

Terms & Conditions

By applying to register a player with the Markham District Softball Association (“MDSA”) you are agreeing to and acknowledging the following terms and conditions:

That you are the parent or legal guardian of the player being registered or otherwise have the legal right to register the player and to enter into these terms and conditions
That you acknowledge that due to the inherent nature of the sport there may be risks involved in the player’s voluntary participation in the games or other activities organized by the MDSA, and you irrevocably release, discharge, indemnify and hold harmless the MDSA, its directors, coaches, umpires and other volunteers from any and all costs, claims, demands, actions or other proceedings including reasonable legal expenses based on any claim, demand or action whatsoever their nature or howsoever they may arise, including negligence on the part of MDSA, its directors, coaches, umpires and other volunteers, resulting from the registered player’s participation in any MDSA activity
That you consent to the use by the MDSA of the player’s photo or likeness on its website or in other promotional material;
That you consent to the collection and retention by the MDSA of your or the player’s personal data for the purposes of registering the player and for providing other communications to you or the player by the MDSA in connection with the player’s participation in current and future MDSA activities
That you will abide by the MDSA Code of Conduct, rules and regulations as published on the MDSA website
Every effort will be made accommodate jersey number requests based but MDSA cannot guarantee all requests will be granted.
In the event of any disruption to our season a portion of the registration fees will be refunded
